Options for completing a fillable form:

• If you have Adobe, download and save the form and use the digital signature function. Save with a new name and return the form to registrar@youlvxin.net.

• If you do not, you can download Adobe Reader for free and create your digital signature. http://acrobat.adobe.com/us/en/acrobat/pdf-reader.htmlhttp://acrobat.adobe.com/us/en/acrobat/pdf-reader.html

• Once you have it, download the form and save. Click in the signature field, choose "Sign with a Digital Id", configure new digital id, continue. Mac will ask you to create Apply KeyChain. Sign the document, save with a new name and email it back to us at  registrar@youlvxin.netregistrar@youlvxin.net

If the form needs other signatures (instructor, advisor, dean etc.), we will accept permission sent from LEC email addresses during this time.
